TERMS / AGREEMENT ON ALL BACKGROUND INVESTIGATIVE REPORTS obtained from Before You Hire, Inc. (CRA):

• All users of the information provided by each report must agree and adhere to the guidelines set forth by the Fair Credit Report Act (FCRA); including but not limited to obtaining a signed Authorization Form.
• Results are based on a combination of the following: Name, SSN, Date of Birth, and Address.
• Identifiers such as Date of Birth, SSN, and Address must not be used to discriminate against subjects.
• All individuals, companies, and other entities that will use the subjects information agree to follow the guidelines of the Fair Credit Reporting Act (FCRA).
